"Rules and regulation from Botox medication guide that was totally ignored by my former pain management doctors!"



This Botox Medication guide was sent to the College of physicians and surgeon along with my forensic test result and  other documents to back my claim that these doctors where negligent. No surprise they denied my complaint and closed my files. I also sent this to the review board recently which also denied a review and closed my file!
To me it was relevant and important facts that was not only ignored by doctors and now also the college and the review board. Talk about being "one-side" like my friend put it so well! She was more upset at the decision then I was. I knew the first respond I got from the College that they wanted to dismiss my review but felt compelled to respond and show that indeed I do have solid bases for a review.
Spent a month working on my evidence foregoing interacting with my friends and acquaintance so has to not be distracted and present the best evidence I could find! Well I can say it was not all for naught cause I am learning so much about how certain people react and what makes them tick.
Their action and reaction when they face the truth scare them. They can try and discredit me and now getting harder for them to do that I will not be dismiss or ignored this easily. They can't intimidate me any more so I decided to post this guide so you can see for yourself if these doctors follow proper  protocol and follow the guideline.
Also this Botox medical guide is supposed to be distributed to the patient before treatment. In my case I downloaded from Allergen site in 2009, after the treatment. By posting it at least information that is important for people to know but not provided by their doctors can access it through here.
I did mention to the college and the review board that there was a antidote that they could have ordered to help alleviated some of the reaction even though it is not a guarantee it would of help in some way. See page 14 "Overdosage" paragraph 3"In the event of a overdose antitoxin raise d against botulinum toxin is available from the Center for Disease Control and Prevention(CDC) in Atlanta, GA". further down the paragraph line 5 it state."In the event of suspected of actual case of botulinum toxin poisoning please contact your local Health Department to process a request for antitoxin through the CDC directly at 1-770-488-7100.
I ask if there was an antidote at the time, I was told "No"and called "delusional" by these doctors! They had no decency if they knew and had access to this guide which they should of then it is a disrespect for a human life. "Ignorance is not always a bliss especially if they know the truth, then lies about it, that is outright stupid" They where trying to send me to another clinic which I refuse since I was worried that they would blame the other clinic for their mistake and incompetent  and I was not going to participate in their plan! I wanted to make sure that the right person has to shoulder that burden!
I will go step by step on how these doctors flaunted the rules and regulation set by Allergen.

Page 2 paragraph 3 it state"The initial dose for a patient without prior use of Botox  should be at a lower dose, with subsequent dosing adjusted based on individual response. Limiting the total dose injected into the sternocleidomastoid muscles to 100 Units or less may decrease the occurrence of dysphagia"
Facts was started on 400 Units in June 2004 eventually raise to 600 in 2007, see blog that shows the amount I received at the time and what the legal amount was from Health Canada which match Allergen! Please see blog,

Page 3 paragraph 4 "Hypersensitivity reactions" it state "Serious and /or immediate hypersensitivity reactions have been reported. The reactions include anaphylaxis, urticaria, soft tissue edema, and dyspnea. If such reaction occurs further injections of Botox should be discontinued and appropriate medical therapy immediately instituted. One fatal case of anaphylaxis has been reported in which lidocane was used as the diluent and consequently the causal agent cannot be reliably determined." Page 4 paragraph 7 "Warning" it state"The recommended dosage and frequency of administration for Botox should not be exceeded. Risk resulting from administration at higher dosages are not known" Risk are known now with more awareness and reporting which these doctors and institution have failed to do and still denies in the face of facts.

Page 5 paragraph 4 "Precautions" it state "The safe and effective use of Botox depend upon proper storage of the product, selection of the correct dose, and proper reconstitution and administration technique. Physician administering Botox must understand the relevant neuromuscular and/or orbital anatomy of the area involved  and any alterations to the anatomy due to prior surgical procedure. An understanding of standard electromyographic techniques is also required for treatment of strabisnus and may be useful for the treatment of cervical dystonia" Did they (doctors) understood this part?

Page 6 paragraph 3 "Information to patients" it state The physician should provide a copy of the FDA_Approved Patient Medication Guide and review the contents with the patient. Patients should be advised to inform their doctor and pharmacist if they develop any unusual symptoms(including difficulty with swallowing, speaking or breathing) or if any existing symptoms worsens."

Page 6 paragraph 4 it state "Patients should be counseled that if a loss of strength, muscle weakness, or impair vision occurs"

Page 10 paragraph 5 under "What should I avoid while taking Botox or Botox cosmetic" it state "Botox and Botox cosmetic may cause loss of strength or general muscle weakness, or vision problems within hours to weeks of taking Botox or Botox cosmetic. If this happen do not drive a car, operate machinery or do any other dangerous activities"

Page 10 paragraph 7 under "What are the possible side effects of Botox and Botox cosmetic" it state"Botox and Botox Cosmetic n cause serious sides effects. See "What is the most important information I should know about Botox and Botox Cosmetic" It has a list of side effects including dry mouth, discomfort and pain at the injections site, tiredness, headache, neck pain, eyes problems: double vision, blurred vision, decrease eyesight, drooping eyelids, swelling of your eyelids, and dry eyes, allergic reaction. Symptoms of an allergic reaction to Botox or Botox cosmetic may include: itching, rash red itchy welts, wheezing, asthma symptoms, or dizziness or feeling faint"

Page 10 paragraph 8 it state"Tell your doctor if you have any of these side effects that bother you or that does not go away!
Fact: I did that and was ignore and called "delusional, liar" and sent out packing and told not to return to the clinic!

Page 11 paragraph 3 "Pharmacokinetics" it state "Using currently available analytical technology, it is not possible to detect Botox in the peripheralblood following intramuscular injection at the recommended doses"

Page 12 in Box marked "Distant Spread of Toxin Effect" it state "Postmarketing reports indicate that the effects of Botox and all botulinum toxin products may spread from the area of the injection to produce symptoms consistent with Botulinum toxin effects. These may include asthenia, generalizes muscle weakness, diptopia, blurred vision, ptosis, dysphagia, dysphonia, dysarthria, urinary incontinence and breathing difficulties. These symptoms have been reported hours to weeks after injections. Swallowing and breathing difficulties can be life threatening and there has been reports of death. The risk of symptoms is probably greatest in children treated for spasticity but symptoms can also occur in adult treated spasticity and other conditions; particularly in those patients who have underlying conditions that would predispose them to these symptoms. In unapproved uses including spasticity in children an adults, and in approved indication, cases of spread of effects have occurred at doses comparable to those used to treat cervical dystonia and at a lower doses."
Facts: All symptoms where reported but ignored and what is worse is knowing this now and thinking they just kept injecting me with a elevated amount even after complaining. They profess I never told them of my severe symptoms. Like I told the College "they where my pain specialist, who else would I go to?"

Page 14 "Overdosage" it state "Excessive doses of Botox may be expected to produce neuromuscular weakness with a variety of symptoms. Respiratory support may be required where excessive doses cause paralysis of respiratory muscles. In the event of overdose, the patient should be medically monitored for symptoms of excessive muscle weakness or muscle paralysis"

Page 14 paragraph 2 it state"Symptoms of overdose are likely not to be present immediately following injection. Should accidental injection or oral ingestion occurs, the person should be medically supervised for several weeks for sign and symptoms of excessive muscle weakness or muscle paralysis."

Page 14 paragraph 3 it state "In the event of overdose , antitoxin  raised against botulinum  toxin is available  from the Center for Disease Control and Prevention(CDC) Atlanta, GA. However, the antitoxin will not reverse any botulinum toxin induced effects already apparent by the time of antitoxin administration. In the event of suspected or actual cases of botulinum toxin poisoning, please contact  your local or state Health Department to process a request for antitoxin thought the CDC. If you do not receive a response within 30 minutes, please contact the CDC directly at 1-770-488-7100."
Fact: I comply and reported to all agency including FDA, Health Canada CDC and Allergen when I started reacting the only thing missing was the doctors was not forthcoming then by not reporting or now" Please see blog

Page 15 paragraph 8"Adverse reactions" General it state "There has been spontaneous reports of death, sometime associated with dysphagia, pneumonia, and or other significant debility or anaphylaxis, after treatment with botulinum toxin"

Page 15  paragraph 9 and continued on page 16"Adverse Reactions" it state There have also been reports of adverse events involving the cardiovascular system, including arrhythmia and myocardial infarction, some with fatal outcomes. Some of these patients had risk factors including cardiovascular disease. The exact relationship of these events to botulinum toxin injection  has not been established."

Page 16 paragraph 2 it state:"The following events have been reported since the drug has been marketed and a causal relationship to the botulinum toxin injected is unknown: skin rash(including erythemamultiforme, urticaria and psoriasiform eruption) pruritus, and  reactions'"

Page 16 paragraph 3 it state: "In general, adverse events occur within the first week following injection of Botox and while generally transient may have a duration of several months.. Localized pain, tenderness, and/or bruising may be associated with the injection. Local weakness of the injected muscle(s) represent the expected pharmacological action of botulinum toxin. However weakness of adjacent muscles may also occurs due to spread of toxin."
Fact: I did test positive for botulinum neurotoxin and there are some symptoms that I am experiencing which is not mention or known at the time. Example loss of smell and taste with chemical bitter acidly taste in my mouth that cause blister and cuts and bleeds at times in turn makes me nauseated 24/7. Also in same blog

I want to bring awareness to this poison so people are more inform and able to make sound decision. Obviously that is not what these doctors and the College want the public to know. It is important to me and I firmly believe the longer they think that they are in control I will confirm to what they say. Wrong all they did is make me more determine and resolved to get this out. I call it a "Breach of pubic trust" when they refuse to acknowledge or report  my condition to the proper agency. What are they afraid of?
